What is a silent failure?
A silent failure is a process that reports success while the intended outcome never happens, so no error appears and nobody is alerted.
In GTM, common examples are a form that shows a thank-you page but never creates a CRM record, an internal alert that fires while the customer's email never sends, and an AI draft that sounds correct and isn't.
They happen because software reports the status of its own step, not the outcome for the customer. A thank-you page means the request was accepted. A green status means a step ran. Neither confirms that a person or customer received anything.
They persist because buyers rarely complain. They leave, so there is no ticket or error to find. In acquisition they are especially costly, since the click, content, or rep time was already paid for before the failure.
Catch them by comparing counts across systems (form submissions, CRM records, messages sent) and investigating gaps, testing outcomes instead of status messages, assigning named owners to each handoff, and making it safe for people to say something looks wrong.
